Dr Yong Thon Hon
Dr Yong Thon Hon is a Consultant Cardiologist with sub-specialised accreditation in interventional cardiology.
He graduated from University of Aberdeen, Scotland and underwent foundation year doctor training in Glasgow. He obtained the Membership of the Royal College of Physicians (UK) and subsequently completed advanced cardiology specialist training in Singapore. He pursued his interventional cardiology fellowship training at Imperial College London where he obtained his expertise in complex coronary intervention and participated in various coronary physiology research studies.
Dr Marcus Chen
Dr David Teo Choon Liang is a psychiatrist with over 15 years of experience. He specialises in youth mental health, mood and anxiety disorders, psychosomatic medicine, and psychodynamic psychotherapy.
Dr Teo graduated from the Yong Loo Lin School of Medicine, National University of Singapore (NUS), and is a Member of the Royal College of Psychiatrists (UK). He is also an accredited psychiatrist with Singapore’s Specialist Accreditation Board. He completed advanced training in Psychosomatic Medicine (Consultation-Liaison Psychiatry) at Columbia University Medical Center, gaining expertise in managing complex psychiatric and medical comorbidities. Additionally, he trained in Intensive Short-Term Dynamic Psychotherapy (ISTDP) and Group Analysis and family therapy, enhancing his capacity to provide integrated person-centred and family systems-oriented care.
Dr Jennifer Guan Huey Chen
Dr Jennifer Guan is a Palliative Medicine specialist and Head of Dover Park Hospice Home Care. She focuses on integrating palliative care principles into community and family medicine practice.
Assistant Professor Teo Hui Ying Valerie
Assistant Professor Valerie Teo Hui Ying is a Family Physician, Senior Consultant and Head of NHG Kallang Polyclinic.
She obtained her M.B.B.S from the Yong Loo Lin School of Medicine and her Master of Medicine (Family Medicine) (Gold Medal). She is also currently a Fellow of the College of Family Physicians FCFP(S) and is accredited by the Family Physician Accreditation Board as a Family Physician.
Asst Prof Teo has taken on several appointments including being the chairperson of the Medication Management and Usage Committee since 2016, driving medication safety, antibiotic stewardship and evidence-based prescribing in Primary Care and a member of the National Pharmacy and Therapeutics Committee.
She is also passionate in Health-Social Integration and has led efforts to improve accessibility of care and integration with the community. She is actively involved in innovation and healthcare transformation, leading several innovation projections and workgroups.
